Introduction: Narrative medicine seeks to reveal the meanings attributed to experiencing illness, within a comprehensive context respectful of the person receiving care. (1) In the course "Education and motivation of health care staff" on the Master’s Course in Palliative Medicine at the University of Turin (2006-2008), written narratives were used to stimulate students to reflect on their professional caring experience in end-of-life situations.
Materials and Methods: Partecipants were encouraged to use written narrations to reflect on and explore significant events in their professional experience in palliative care, and then share them in small group and plenary sessions. (2)(3) The aim of the methodology is to develop an approach to and an appreciation of problems not according to a predefined paradigm but rather through a dynamic process that explores experiences from a clinical point of view (e.g., to deepen understanding from an inside perspective, uninfluenced by pre-codified knowledge). The narratives were analyzed using a phenomenological approach, and the units of meaning emerging from written texts were identified. (4)
Results: The dimensions identified in the 25 stories were: accompanying the dying person and their family; and the changes this experience brought about in the health care professional. The themes for reflection were classified as: courage and will to live until the end; sense of limitation; need to protect and be protected; perception of one’s own feelings.
Discussion: The importance of developing reflective and interpretative skills (5) was drawn out by asking questions about the people in the story: who they are and how they behave; what are the doctors’ attitudes; (6) what is the meaning of the story, what feelings does it arouse, how does it impact on the way of working, also as a form of "learning on the job" and by discovery. (7) (8)
Conclusions: The narrative method enabled students to develop reflective thinking on how they express themselves and interact with others, and on their professional attitudes towards death, and thus helped them to better direct their attention to the "personal story" of people cared for.
Reviewer's comments on the document:
