"The identification of nursing diagnosis (ND) and care planning are becoming increasingly relevant in the work of nurses in primary health care (PHC).
Objective. To find out more about the use of ND in particular and the Nursing Process in general for nurses in the Area and to ascertain which ND appear most often in PHC.
Materials and methods. A cross study, carried out in 39 Primary Health Care Units in Madrid’s Área 11 in 2001.
Both quantitative and qualitative evaluation indicators were designed. Specific nursing diagnostic records were reviewed for all those involved for the quantitative evaluation and a sample of the 1918 case histories for the qualitative evaluation.
Results. 75.5% of the nurses had carried out some form of ND. The total number of ND identified was 5129, representing 13.8 per nursing professional, with a high percentage relating to the individual’s biological sphere. The “Global Quality” indicator, which evaluates the following through of the various stages of the nursing process, was fulfilled in 78.6% of cases."

It is vital to have a specific information system
for Nursing Care Plans providing information on a
fundamental Área in the service provision offered
by nursing professionals in PHC.
