"Objective: Know the situation of social support with a social program at home in the community.
Subject and method: Observational, descriptive, prospective Study in January 2001 -December 2002, with the dates analysis in a first part of longitudinal study to predictors factors needed at home in a program attention residence.
Results: 78 subjects included in the study until December de 2002. 47 women (61,8%) and 29 (31,8%) men. Means age in the first year 80 years and 2002 de 78,5 years. Hospital length of stay 58,7. The mean Barthel index at admission 41,5 and at discharge 75,4 in the
unit care -rehabilitation.
The 34 subjects included in the home care program in 2001: one patient go to long care for pathology, disorders and dependence- Two were derivate at agut hospital for agut process. Three patients was exitus. The temporality of these program (SAD) in a 60% the persons were in the period 2001, and after can be discharge of the same. At 2002, in 6 patients (14,2%) need admission in the unit care. Two patients were readmission palliative care,
two in long term and two in the psiogeriatric unit. Conclusion: The SAD is a program has made possible of the patient during more time at home. In these way, the institutionalization can bee later, receiving a sociosanitaire support. The family is principal nexus of attention for these patients.
Is important to know the disponibility and proximality the professionals, who require more attention in a situation of stress."

This is very interesting as fas as present the experience of a home care program and how it helps the professionals to reduce the neccessities for the patients involvelt with regards to the hospitalization in aguts hospitals.
It stressess the possitive aspects of this experience related to the improve of the health and quality of life of the patients and also of the non-professional carers, the relatives of those patients that usually are under stress related to the demands and needs of the situation.
